Need help - Active Quickslot issue

I am on PC/NA.
I have an issue with one of my characters, doesn't happen to any of my other characters.
If I make a consumable, outside of food and drink, my active quickslot, the wheel immediately replaces it with a food or drink.
On the gif below, Essence of health, Essence of speed and Cyrodiil repair kit are all in turn replaced by Lava foot soup, and removed from the wheel altogether.
Did I mess up some game/addon setting on this character to cause this?
None of this happens on any of my other characters.
