[U48] Quest stages completing spontaneously

I believe I have found a serious questing bug; I was unsure until I just witnessed it happen again - quest stages are sometimes completing spontaneously:
- Axes for the Assault - progressed from "craft 3 axes" to turn in phase "talk to armorer Reistalf" without any crafting completed.
- Masterful Jewelry - I've had it uncompleted for months, it spontaneously advanced to turn in phase without any crafting completed (I can't even craft a ring with 9 traits, which is why it's been stuck).
- Capture Any Nine Resources - messages started popping up every several seconds, updating progress by one each time until it switched to turn in phase; this happened in Solstice, not in Cyrodiil, and I wasn't grouped with anyone.
I believe I may have witnessed the same with the vitrified souls quest on at least one occasion.
Possibly related, two daily writ quests I've had before the U48 patch, enchanting and jewelry, are now stuck in uncompletable state, not reacting to any crafting (the quest prompts to craft tiems are greyed out like failed stages).
