I can answer this. Had a friend run into one before completing the prologue on PS, so I can confirm the spawns aren't affected by that. ^^
I'm well past 100 dark fissures, over 4 hours of my life, and zero reaper spawns. gg @ZOS
Edit: One thing I have noticed is that previously Dark Fissures re-spawned about every 5 minutes, now only some of them do. The others seem permanently inactive.

My guess is, that they act like fishing holes. If you emptied a hole, another hole will randomly spawn in the next 5 mins somewhere on the map.
Could be the same with dark fissues too.

I spent a straight two hours on the Stonefalls chain with no luck. If it is hotspots it is very poorly thought out. If players cannot predict when content will spawn, it makes doing content near impossible for the vast majority of the player base.

First fissure was Worm, and got a soul fragment!
Funny how a bug related to a paid item is quickly addressed while this isn't even aknowledged after weeks.
Yesterday, after the patch on PS-EU, Soul Reapers spawned regularly. My group and I found about 30 within about two hours, in various areas.
What was noticeable was that far fewer spawned with a boss at the end that drops a fragment. Only about 6 of the 30, or about 20%.
Of the 8 Soul Reapers I found before the patch, 6 out of 8 spawned with a boss.
What I noticed is that there's a connection between the NPCs that are attacked by the Worm Cultists at the beginning and the boss spawn.
If there are only two NPCs, no boss appears.
If there are three NPCs, a boss appears at the end that drops a fragment. This was the case with all 6 boss spawns. A boss never appeared when there were only two NPCs.
So you can predict in advance whether a boss will appear.
