Survey For Performance Can Only Be Done Once

Thanks for getting the survey setup!
I filled the survey out for what I found to be my most common issue, but I've had all of the issues that the survey listed. Is there anyway that the survey can be enabled to be filled out more than one time?
Hopefully we can get as much info on these issues as possible!

I also had all of the issues that were listed so I selected the one about the ping spikes and then in the comments said something how I get ping spikes and during that time my skills dont fire, I get rubber-banding, and sometimes crash.

I don't know if this helps or not or was the right approach.

@ZOS_Kevin I would adjust the survey so people can select more than one (or all) options, if that's not possible it would be good to add an additional option that's something like "I am experiencing multiple of these issues."

I selected the rubber-banding option as that is what has affected my raid groups the most in the past few months, but we've all also had the input lag and random disconnections too but we can't select the options for it.
