Critical Surge - Bug

On numerous occasions now my Crit Surge bugs out randomly and removes itself. It's been doing this for the better part of a year now in any and all fights I get into, I cannot sit watching my bar to see if it bugs out and removes itself and stops my healing for me to reapply it.
Could it be me:
It's on my backbar - Paired with a bow.
No I am not getting hit with negate magic as its all fights.
It's typically around the 13 second mark when the skill lasts 33.
This is Xbox EU server.
