Just summoned Mirri who I haven't used in a long time, and she is in her underwear and all gear removed, nothing to equip. I know I had her in a full set of heavy armor a few months ago. Also most of her skills are reduced to 1.
Don't know if this is after the recent patch or not as I rarely use her.
All other companions are OK.
