Is the occurrence probability of "Supplemental Thread" too low?

I've completed 90% of EA's "Exploration" achievements, but "Supplemental Thread" never appeared, preventing me from completing "Seeing the Big Picture." Is the occurrence probability of "Supplemental Thread" too low? Or is this just how the game is designed?

    I've killed myself twice at a boss during my arc 1 daily runs, and it eventually showed up. I didn't die, and I start with 4, so I had 2 at the time. I don't think you have to be at 1 thread, specifically, but I'd wager you have to be at less than your max.

    Thanks for everyone's help. When I deliberately saved one life, I successfully killed the BOSS and obtained this achievement.

    Oh, that makes a lot of sense! I was wondering why this was never showing up, but I always end my runs before I run out of threads. Will strategically die so I can get it for the achievement, thanks. :D
