Looking for "true" RP target frame addon

I am currrently using Azurah for the sole purpose of removing extraneous information from the player target frame. Unfortunately, this allows me to only remove the account name. While there is also an option to remove the alliance indicator, this does not appear to work, and there is none at all for the level and the champion icon.
I would ideally like to show only the following in the player target frame:
Does any addon exist that would allow for this?
