The perk does become account-wide, although it takes more than getting them to level 20. You need to get the multi-part achievement that also includes maxing rapport and completing personal questlines among other things. (This is Bastian's for reference.)

the keepsake has more requirements than just the companion level

as noted above, the other requirements including achievement for maxing rapport, achievement for doing the companion quests, achievement for doing specific +rapport activities (and for older companions, also equipping at least 1 purple companion gear item, this requirement was removed from azanrder/sharp)
