Galen's Dream of Kasoryan quest not drop ToT fragment

I already finish all Galen main story, including the "dream of Kasoryan" and just got the info that a Tale of Tribune deck fragment was supposed to drop after finish the quest. My fragment menu is empty and it says the quest was not done yet.
I did Galen complete story lines with 2 characters already and the fragment remain absent, saying that "Dream of Kasoyan" need to be completed. I ask please help in this situation.
