Kyne's Aegis Trifecta Achievement - broken ?

Hello,
We were running veteran Kyne's Aegis Hardmode on 13.07.2023, where we achieved full vitality (36/36) and speed run (19:35), with esologs and recorded video to back it up.
However, nobody in the group got the no-death (Perfect Strom) or trifecta achievement (Stainless Siegebreaker).
The score we had was 242551 which adds up in the score calculator.
We also compared score before entering Falgravn with other trifecta run, and they were exactly the same - meaning we didnt skip any adds.
We of course checked all corners to see if there is something that we could left unharmed, but we couldn't find anything.

Is it possible that incremental patch on Wednesday appears to have broken these achievement in KA ? it's the only thing that's changed.
What we'd like to achieve with this is to know whether we got really unlucky or if we can expect the same thing to happen on our next 36/36 vitality run in the future. Because that would crush our spirits.
