Quests that I have completed are showing as "incomplete" in Zone Guide

I am having an issue in my game where quests that I know that I've completed are showing as incomplete in my zone guide. I had completed all of the zone quests for all of the zones (except Apocrypha/Telvanni Peninsula) on this toon, but my zone guide is showing that I've only partially completed them.

Just noticed I've also I've lost completion in every zone apart from Cryodiil and Galen as per the zone guild/map icons, even lost from the recently completed Telvanni Peninsula & Apocrypha zones.

Please can this be rectified to show accurate completion.
