I wonder how many are experiencing this bug or if we will ever see a fix.
I've contacted assistence and provided all screenshots and proof this is not related to an addon. Sadly the last message i got asked me to try "reload ui" lol.
Screenshot here to show more clearly that it's bugged.
1) I have dug up 10 fragments for a complete music box (Found: 1)
2) I have dug up 2 more fragments after that (12/30 codex)
3) Is where it gets strange because the overview says I have found 7/10 pieces and
4) The Inkwood box is the first one in the list that shows I still need to find it, as well as the four pieces after that, meaning 5/10 missing.
The game cannot math, it seems.

You’re better off talking to a wall because CS responses rarely make any sense at all.

Which it obviously is btw because people from multiple platforms have reported this issue. It's a multi-platform bug, I'm on Xbox EU so don't even have access to add-ons. This is not an add-on issue.
Btw just to be clear, this isn't me moaning about lead drops and RNG. This is an actual issue. On the "Music Box, Glyphic Secrets" - I completed one full music box. Since then I've legitimately gained two leads (Intricate Winding Shaft and Inkwood Box). However when I select this antiquity in the antiquity menu, it's all over the place as to which fragments the game thinks I have etc. I know many other people are experiencing this too. Zos is always trying to get people to use systems like Antiquities - would be super great if they were actually worked properly. Thanks
It's a well known global phenomenon across antiquities, that the *first* lead is always a lot more common than the second, if you should happen to need the same lead three times for its codex factoids.
So it's hardly surprising that you get all 3 leads for the well once, reasonably quickly, and then they become a lot rarer when you're trying to get the bits of a second well.
Happens with other mythics, even other single golden Master and Ultimate antiquities too. For instance, the Golden Idol of Morihaus - the first one arrived very quickly, I've been killing minotaurs for literally years since then and not got a second one. It took me two years to get a second Branch of Falinesti, and a second Rune-Carved Mammoth Skull... But then the third one seemed to come along reasonably quickly in both cases.
The annoyance for me, is the sheer extent to which this is the case - the difference is WAY too huge.

Look at the positive side at least you don't have to spend days to convince support is not an addon problem.
Sadly we are losing a lot of hard to obtain leads because of this bug.
I was talking about the Apocryphal Well leads, which appear to be an extreme example of an already documented general phenomenon (namely, the extreme rarity of second and subsequent gold leads of a given type) - I personally am unsure whether this is a bug, or as designed, but either way, if it's deliberate (for repeating second leads to be this rare), then it's *bad* design for them to be THIS rare.
The music box issue is a separate issue, the result of *this* being that I refuse to even attempt to complete my first music box until it is fixed. The game should not be crediting us with repeated leads (e.g. five "free" leads in the Music Box) that we have NOT actually found: for a start, it messes with the whole codex entries thing, and for another thing, it messes completely with people's heads.

Should I stop getting the second leads for the ones I am able to in case there is a complete roll back of all leads? Some response from ZoS would be great. This is where I hope to see someone acknowledge the bug when ZoS asks "How can we communicate better".
