Missing Achievement?

The Q1 Dungeon DLCs typically have an achievement associated with them; a 5-pointer for entering either new DLC dungeon for the first time, which grants a Collectible reward of some varying variety. These have titles of "<DLC Title> Explorer;" e.g., the title for this DLC's Explorer achievement would be "Scribes of Fate Explorer."
However, currently, there is no Scribes of Fate Explorer achievement.

Is this an intentional omission/change to Standard Operating Procedure, or might it be an oversight?
Also, as long as I have your eyes for a moment, I noticed, while looking for the above achievement in the Journal, this [I'm guessing] extraneous "template" achievement:
