NPC at end of dungeon/trial

How do you complete the final NPC story of a dungeon/trial for skill point/loot if the game drops you before you can reach there?
For example, the Wrothgar dungeon where you need to speak with Lyris?
In some trials you can just relog and go back to the beginning and see the NPC there waiting to give you credit.

as far as im aware for dungeons, if you say crash and the instance is gone (everyone left the dungeon) you would have to re-run the dungeon fully because the turn in is at the very end

never heard you could do that with trials, that would be new to me, but in those cases ive never forgotten to turn in the quest (usually we dont finish the trial, or finish and turn in and people tend to hang out in a trial longer exchanging gear)

The trials are different as some give you a trinket or object to turn in for a quest not picked up or not turned it. Most dungeons require you to run it again. Fungal Grotto 2 and Elden Hollow 1 have the NPC at the beginning (which is the end of the storyline) but I can't remember if leaving the dungeon resets the quest.

From time to time, you will encounter the NPC not appearing at the end. This happens a lot in the Dread Cellar. Sometimes reloadui works, other times, traveling using the portal works, and others you have to travel out and travel back in to a group member still in the dungeon to get the NPC to appear.
