Big fix that needs addressed is the in-combat mechanic. If you die you shouldn't be in-combat when you resurrect. Also fix the wayrest sewers incombat that prevents you from going thru the door. Been complained about since launch 8 years ago.

    There is an annoying "in combat" thing at the end of Volenfell iirc. And there is a glitchy door in Tempest Island. It is surprising these things don't get fixed, but they are little things really.

    With the wayrest sewers one...if it is where I think it is...you just have to jog back and clear the rat king and his rats that you left in your wake.

    Rat guy is later on. It's the crocodiles in the starting area that hold aggro but can't escape the sewer that prevent you from using the door. I always made a point to kill them so that we can use the door timely.
