Defile may not have huge importance in PvE but in PvP is really important in lowering healing potential of other players.
Removing that aspect would not be good. There's a lot of Statuses that are good in both PvE and PvP, it's ok if some are more focused in one or the other.

i feel like diseased should probably have a DoT portion or maybe just deal more dmg since it's effect is effectively useless in pve.

yeah it should keep defile, that part shouldn't change, but i think it should get similar or the same treatment that hemo got.

The inspiration is a status effect in game called "Wretched Pollution" which is a disease themed effect. Keeping the healing debuff definely makes sense. I like to let other posters make that argument then sound like im reaching in a suggestion. But id love to see an addition similar to how bleed and frost got.
