I've had this since I start playing.. I notice it happens mostly when I loot something then go to sprint away. Doesn't matter how many times I click to sprint, it won't do it until I stop moving and try again.
I'm loath to post it here, because it seems that once the players find a workaround ZoS treat bugs like they're solved, but this works for me: after looting or interacting with anything, always move forwards for at least a second before clicking sprint. It works the majority of times, although I do get caught out now and then. Still, better than happening all the time.

Just to manage your expectations, this is a minor inconvenience to a small minority of players - the majority on PC are K&M, so won't have this issue.
For your own sanity, I'd treat this as a feature from now on..

I do not notice this. I have an Advanced controller with 2 programmable buttons underneath, which I have set as L-stick R-stick, in order not to ruin the analog sticks by constantly crushing down on them. Sure I notice I am not running every time I attempt to press that button but, I also realize I do not always press it completely. However, while I was testing to see if I was effected by this issue, I did not notice any issues since I was making it a point to press that button all the way in.

I've played with nothing but a controller on PC ever since I started and I haven't had any issues with a sprint bug...
By no means am I claiming it isn't happening, far from it, but I was wondering if you're using a wired or wireless controller and if there's any correlation with the controller type and the bug, also, is there any third party controller software being used on PC?

I do that. Do you think it is possible the game is not properly giving priority to the stick-buttons while the stick is not at center?

I know, at times, if I just give the button I use (lets call it L3) a quick tap it may not sprint. But, if I ensure to give it a good squeeze it always sprints.
Like I said, I use an "advanced" controller. It does not have the 4 paddles underneath, just 2 buttons. I got tired of damaging the sticks by constantly pressing down on them. It's a wired Xbox One advanced by PowerA.
It may be that separating the action from the stick helps.
In any case, it does sound like it would be the game not accepting the input for some reason. Reporting that it takes multiple presses my be a sign that holding it down may help. Tho I have not noticed anyone mentioning trying to compensate for a delay. I don't know. I have not tested it. I sort of refuse to squeeze down on the sticks.

Oh, I failed to notice for the PS4 controller its the shoulder buttons. Hmm, well that sort of wrecks my theory.
PS: for PC users.
I made an addon to solve the slow look speed, just look up my name.

As I mentioned... when this bug occurs on gamepad... and it doesn't occur all that often... when it does occur you have to quick-hold the sprint button (in my case PS4 right shoulder button) THREE TIMES and sprint will activate on the third time. OR... you can simply release the forward stick then press it forward again then sprint button and it will then work. For whatever reason, releasing the stick and reactivating it again also 'fixes' the sprint bug... but I find it easier to quick-hold three times instead of stopping my forward momentum.

I think the person who mentioned shoulder buttons is not using the default template, as that presses the joysticks on PS
