Does this proc with Rally? And if it does, does it proc upon cast, or upon heal?

It does proc with rally, but it procs only upon your health bar going up. Meaning you don't need to be in combat to proc magma. If you took fall damage, then single target healed, magma would be active.
But it ONLY activates when your health bar goes up, it doesn't work when you try to overheal yourself

Can probably just get some fall damage or run a higher stage of vamp

How about a bar swap when you have unequal health?

Does minor toughness proc it?

I think in both cases your "percentage" of health doesn't change. So even if you gain 5k health, you might remain at x%, so no proc.
