The Dreaded Trifecta Achievement Bugged

My group and I managed to complete The Dreaded trifecta achieve in The Dread Cellar and we did not receive the achievement despite not dying and being under 25 mins.
We ran through the dungeon at the end to make sure we didn't miss any adds and even killed the passive stones in some of the last rooms and still did not get the achievement.
We know for a fact it is bugged because one of my dps did not also receive the no death achievement (Stay of Execution), and we did not have any deaths as we were resetting the dungeon immediately after we had a death. I also had the same issue with the no death achievement on a separate character who also did a no death run off hard mode a week ago. My group also once again ran through the dungeon to check that we got all encounters and we did kill everything.
I have spoken to friends who have gotten The Dreaded too and they have also encountered this bug in the past.
I was thinking it might be the adds on the first boss that disappear if you kill the Scorion before the adds, or it could possibly be some adds in the portals on the last boss.
This is just extremely frustrating and seems to be a common theme with the newer content as I have heard cases of people not getting Planesbreaker from vRG due to a missed add as well.
I have submitted a bug report in game but just want to bring more attention to this and see if any other groups have had the same issue.

I have not heard back from support yet on my ticket but me and my group did another successful run of The Dreaded earlier today. We decided to record it in case it bugged again- and surprise! It did. Our run was under 20 mins and we used the remainder of the time to run through the dungeon and check for any missed adds that may have clipped through the wall or teleported somewhere. I also show at the end that the achievement is still locked in my journal.

We ran it a third successful time and got the achievement. I also see they are fixing this bug next patch allegedly. I still feel very frustrated we had to do this 3 times to finally get the achievement.@LadyAzura | PCNA | youtube.com/@Azuraaaaaaaaa0
