Because ZOS need incoming money to get the game going, and appearance changes are part of the products selling best. Don't forget that ESO is not a pay to win game. They can earn money only via selling "nice to have" stuff.
Most MMOs let you spend a few gold to change your hair style, facial hair and color. Especially for styles you own or in the creator. IMO ESO really need to step up here to the industry standard.

In Fallen Earth (RIP) you could just wave your hand and rotate through all the hairstyles in game.

The difference with WoW, for example, only charging ingame gold for cosmetics is, of course, that it is subscription-based. Once you drop mandatory subscriptions there has to be an alternative source of funding by way of a cash shop. It's then purely down to whether the cash shop sells cosmetic items or Pay to Win items.
