[MGF HM] Crate doesn’t spawn. Bug?

My group has run into an issue this evening, we were trying to complete Moongrave Fane on HM, but whenever there is the phase where you need to move the crate for the dragon npc to hit Grundwulf, there is a high chance the crate doesn’t spawn at all, or hangs WAY high in the air. Or, only one or two players are able to see it, or it spawns less than a second before the dragon does the fire breath.
Needles to say, it makes it nearly impossible to complete on HM - if we don’t move the box, Grundwulf does an AoE that most likely wipes the group.
Did anyone encounter this? Thanks for any advice.
