Momento causing whole group to crash PS4

The Bone dragon summon focus momento is causing group wide blue screen crashes when used on PS4 NA server. The PlayStation error it lists is (CE-34878-0). It happened every time the momento was slotted then used. We tried it in both trials and dungeons both, and it occurred 100% of the time in all cases.
