Skills swapping in dungeon.. bugging out and have to leave game..

When in the skills menus in dungeon.. if your group runs ahead and starts fight.. the game bugs out and gives you an A to dismiss.. hit the A on XBox1.. nothing..have to restart Xbox to get back into game..
