The annoying 4th group member that doesn't hit "ready"

We've all seen it. The random dungeon group times out because one member doesn't ready up. This weekend I got to be that guy. I was playing in the same room with another party member. This time I was the tank. As I'm waiting for the queue to pop, he says, "Are you going to hit ready?"
I say, "I don't have a prompt."
He says, "Everyone else has readied up."
Sure enough, on his screen the party is waiting on a tank, on mine, no prompt, no notification. The message in the quest area still says I'm queued, and when I go to look at the party menu, the game kicks me. I wasn't porting, my internet wasn't unstable, the other player using the same router didn't lose connection or get kicked from the game. When I logged back in, we were able to queue almost immediately with no further problems. Go figure.

Just in case, it sometimes gets minimised into the notification icon on the chat window if you're in combat and doesn't show as a prompt on the screen, and needs to be clicked on to be brought up.

99% of the time, it is the DPS. They wait 30 min on the queue and just when they go to the bathroom, their turn comes up. When they come back, it is already to late.

It's always the DD! Used to get annoyed until I realised it isn't their fault, given how long their queue is. Also, statistically they are more likely to miss it, since there's two of them

DPS queue is long and boring at time. Go off and a harrow storm or something similar and miss the notification.

It keeps happening to me while sitting at dolmens on Xbox. Is there a way to bring it back up once it disappears as I can’t find it?

I use auto accept.

The most annoying thing is that when the 4th (dps) hangs sometimes i simply loose the queue and have to start again.

Plus if you've gotten distracted and you're doing something out of game like checking your email and stuff (going to the bathroom, making a cup of tea, etc etc etc), the queue popping notification isn't really that loud or that distinctive. I wish they'd change it.

On PS4, if you bring up the menu it often goes into the notifications section if you're in combat and don't get the on screen notification.

It's sometimes bugged when queuing with a pre made group, we have the same issue as the OP.
When using the dungeon finder with a friend it happens that one of us gets the ready notification and the other absolutely nothing. Also the on screen status still stays "Queued" while the other can see the group ready status ... until timeout.

I ran probably a hundred random normal dungeons or more in the last few weeks because of the XP bonus, and probably at least 10% of the time this would happen to me. I queue solo (NOT in a group), notification pops, everyone accepts but one, and eventually it times out and I'm kicked from the queue. Why am I being punished when I accepted?

+ the annoying fact, that ending up in a group, that has already started the dungeon, doesn't let you complete the daily pledge quest anymore. So it's a complete waste of time.
