Grymharth’s Woe bug - frequent bluescreens when trying to exit (PS4)

Title says it all. I own Grymharth’s Woe, and often when attempting to exit the home, the game bluescreens. Any type of exit can trigger it - leaving out the front door, the upstairs exit, or by trying to teleport out.

Forgot to mention - has even happened when trying to switch characters if I am in the house when trying to switch.
