QUEST NPCs KEEP DISAPPEARING

Currenlty in Valley of Blades main quest - there is no Sai Sahan, although i do need to speak to him to progress.
Also have a quest Over the Edge in Reaper's March - the witness to murder i nee to speak to is just not there.
Same keeps happening to NPCs around towns - they just bleep out of existence falling through the textures.
Impossible to go through the quests without restarting. Pls fix.
