When you get rushers and snails on the same group, you may get this problem. If someone is still fighting enemies near the start while 9thers start fighting enemies at the door area, the npc gets stuck at the starting area.
You still hear the dialogue from the npc after clearing the room but door won't open because the npc isn't there to carry out the scripted action to open it.

IMO it is almost always a single crocodile or other add stuck in the canals somewhere. And that's not a problem of rushers or snails, it is ZOS' problem that they haven't found a way to break up the "in combat status" after a given time. It's just silly after all these years.
That door has been broken for literally more than 5 years. It should be an easy fix to force it open once the miniboss in front of it is dead since we have that mechanic in many other dungeons. I personally think a game should not try to contend for the "labour of love" award on steam without fixing issues like this for years yet here we are..
