Vampire feed skill barely works, doesn't work on all npc's and only works at specific point

Almost have the NPC's I run into in the game don't let me feed and most of the others you can only feed at a specific point, why this is doesn't make any sense, are vampires that picky that they only get hungry when looking at someone's back at a specific point? Are is the lore that they can only feed now at that point because of an inadequacy of their abilities? I really want to love this type of character, but this one ability just feels even more broken since things were changed with them.
