Crashing in Trials

I was just in a vHoF run, and during the Pinnacle Factotum fight, multiple people would just crash. Their games would either hard-freeze, or close completely with no notification. I have had friends say the same thing for either trials, and it is very inconsistent and seems like some people get a good instance, and others get a bad instance. What happened in the last patch to re-introduce all these issues that were said to have been fixed before?

It's horrible, one moment it seems fine and the next moment it just closes the game without any warning (or the annoying 'whatcha doooin?' pop-up).
