Happened twice today in a matter of minutes. I was doing a main quest where you go to a tower in Coldharbour. Both times were with the same NPC. He was a shielded enemy, and did his charge attack. Both times, he ran through a wall, and I was unable to do anything to him. I had to wait for him to clip back through to keep fighting.
