I've come across a bug 2 days in a row now in the final fight with Molag Kena.
She will call down the 4 ghostly figure things that hold her shield up, and one of them will die, but not, and stay there holding the shield up and be completely untargetable. You're thus unable to defeat the boss.
Fix is to wipe and do it again.
