Moongrave Fane - Risen Ruins Bugged

There is a persistent bug (besides the reset one) that when the Risen Ruins does his charge attack (or the ground pound), if he runs over the top of you (when I have him held/taunted), more often than not he can smash you through the ground, and put you a little bit more than 6ft under..

Like ive had once or twice where I've landed on a rock a body length underground, and twice where ive just fallen into the perpetual blue eternity below.. I'll post a screenshot when I get home but yeah its getting annoying, happened 3-4 times already in the last week alone. death through falling through the floor, and not being able to loot when I res and port back over...
