Given that it is off GCD, I can easily see people working it into Macros or adding it as an additional step when weaving to still get the buff at 50%+ uptime while avoiding the ramping damage per second.
Rather than it costing health while active, they could have made it cost primary resource while active. It would then be impossible to sustain through without some serious building towards it, but would still be possible to use for a burst. Kinda like an overcharge ability.

They have some sort of fixation on Vampires harming themselves to deal damage, which is thematically opposite of everything that defines vampirism in general, let alone in Elder Scrolls.
