End of Campaign Rewards

On PS4 NA nobody recieved campaign rewards. I was told by someone that we were supposed to file a ticket to get the issue resolved but from everyone I know we have yet to get an official response. I didn't even receive an automated email response.
Did anyone have success getting their rewards yet or am I doing something wrong?

Did you actually get to tier one to qualify for an end of campaign reward, @codierussell?
