NPC audio doesn't load when entering new instances

There's an ongoing conversation about this in General Discussion, but I figured I'd raise this as a bug report too.
I'm playing on the PC, EU server, but as you'll see below this seems to happen on other platforms.
Essentially, there are situations when entering an instance where dialogue audio for NPCs isn't playing, but subtitles for said audio appear.
See 3 examples below:
Zamarak's first line entering the Dragon room (Elsweyr tutorial)
Here the audio loads (20:00):

Here the subtitles aren't active, but you can see (or hear) that Zamarak's line doesn't play (34:11):

No audio for the subtitle when entering in the final room of Soul Shriven in Coldharbour
Here's an example of it happening on PC (Elsweyr update, so very recent). I did find videos where the audio plays, but they all seem to be older versions of the game. (01:16):

Here's an XBox example, so this suggests it's a game bug and not related to PC hardware or drivers at all. Soon as the instance loads the subtitle appears but no audio (17:30):

Captain Carvain line never plays audio when entering the Star Haven Catacombs
Might just be because the NPC's not actually in the room (09:09):
