Master Alchemy help

I have to craft a Masterful concoction with the following properties:
Essence of ravage health, gradual ravage health, ravage health, ravage stamina
I have tried over and over to make this but I can't find the right combination of ingredients.
Can anyone tell me the recipe please?

pretty sure that you cant get a poison with 4 effects, only 3.

Yes, @OG_Kaveman, it is a master writ ... but not a poison if they’re making an Essence of Ravage Health.
