Instant death bug and then I can't resurrect.

For about three weeks now in battlegrounds my character will just randomly drop dead and then I can't res. I get "E to revive in place" "R to revive wayshrine" and no players on my team can resurrect me. I have to leave the battleground and get a debuff. I haven't been able to pvp for weeks.
My death recap hasn't worked for at least 3-4 months.
Before this in battlegrounds I randomly fell through the floor and my character couldn't cast abilities until I died.
I can't weapon swap in pvp sometimes and if I do there's a huge delay always.
I haven't been able to see my helmet for months now when it's turned on.
My weapons disappear and sometimes they end up in other weapons slots.
Recently when I log in, my character's old costumes are equipped even though I didn't equip them.
These are just the main ones that I care to list right now, there's plenty more.
