Wrong ultimate activating

Since a few months, I frequently get an issue with the front bar ultimate going off instead the back-bar ultimate while clearly on the back bar. This is not due to messed-up bar swapping, the UI and character model clearly show the back bar. Front bar ulti is Temporal Guard, back bar is Life Giver. It's not restricted to laggy prime time situations.
Just would like to know if this is a common issue experienced by everyone.
Also, when it's really laggy, my character occasionally uses the Radiant Regeneration animation. I unslotted this skill weeks ago. It's not casting the skill, but the animation and the sound clearly play.
