Daggerfall was notorious for it, was so easy to get into the void it was an tactic to do dungeons fast.There's an entire wall in Summerset (the secret room where the mage girl is hiding) that you can just jump through and fall into the eternal ether!
Not a solid bit to it.
There's a number of places that you can fall or be "knocked" through in the game.
Same thing existed in previous ES games (notably... skyrim), so it's definitely lore-centric!

Crypt of Hearts especially. I spent several minutes fighting a mini-boss on the wrong side of a wall with my tank (hence the slow fight) before I was in any position to get back in. I was actually surprised how well the system worked in that non-standard situation.

Welcome to ESO since release. This happened to me multiple times, at least once or twice a week. Last time being a few days ago while doing pledges. Normally happens in dungeons (looking at you Crypt of Hearts & Wayrest Sewers). Seems to occur when knocked back or more commonly when feared into walls by enemies.
