Unable to complete Banished Cells Vet achievement (with screens)

Boss lady spawns as normal not vet (VERY obviously as you can see her HP and she drops like a stone).
My friends and I have done this multiple times now and cannot get her to spawn in vet form so it's not ticking off on our achievs.
Even though she's not spawning in vet form she is dropping purple items.
We have all put in bug reports via in game /bug option.
