As of the Elweyr patch, I've noticed that a large amount of skills that are cones or melee ranged do not register.
Noxious breath and carve are two examples of this. You'll see the opponent just get covered with noxious breath, all up in the animation, but it doesn't register a lot of the time. The same happens with carve all the time that leaves you jokingly saying that your opponent must have ducked it lol.
Venomous claw and burning embers love to pump fake, as does volatile armor. The animation will fire but the skill itself will not be casted. It's a real pain in the arse having to constantly double or triple mash volatile armor to get your armor buff. I have also noticed that when skills "Pump Fake," they still consume the global cooldown.
I've used the dragonknight class as the primary example but it happens with other classes and abilities in the game.
This needs to be fixed to make combat smoother and more enjoyable.
