Clarification on Stealth mechanic

Hello, that might be a dumb question, and I should know about it after 4 years playing the game, but even though I have a Champion Nightblade, I really don't know how to play it.

How does stealth work?
What breaks it, what doesn't, is there a little delay that prevents you from being pulled out of stealth when using cloak?

I main stamsorc, I do some PvP but not much, I usually don't bother with detect pots or Expert hunter and morphs as I have Hurricane.
The other day I encountered a NB in BG. This player was making really good use of stealth, and didn't let any opportunity to fight back, they were periodically attacking and cloaking away, staying visible for less than a GCD each time, at least that's how it was on my screen.

So, I tried running toward the general direction of this NB with hurricane up, nothing. In general, it sometimes pull out of stealth, but not right after the NB cloacked.
When using DB, my screen displays "Missed, Missed, Missed...", I know that means my hit landed, and the NB was taking a DoT, but was still invisible.

On the other side, I know causing any damage pulls you out of sneak. So for exemple, If I wanted to make a stealth build on a class without cloak, I should make sure I only have direct damage and no DoT so I can crouch after attacking to get back to stealth.

So, if you have any clarification on the rules about stealth (with and without cloak), it's welcome, as well as any tip to fight against or play a stealth oriented NB, or non NB builds utilizing stealth (I for exemple would love to try some OS gank builds on my khajiit stamsorc)
