Murkmire's The River of Rebirth achievement did not unlock

I'm playing on PC-EU.
Recently (about 3 weeks ago) I completed Murkmire's final quest, called 'By River and Root'. I did get the quest rewards and the map shows that I have completed all of the main quests as well. (see attachments)
However, the corresponding achievement did not unlock. This unfortunately makes it impossible for me to get the Hero of Murkmire title.
I have tried rebooting the game, returning to the quest ending point, talking to the NPC I got the quest rewards from, and waiting for a couple of weeks. Nothing seems to work but I'd love to get the title. Does anyone have a suggestion?
