Dropping dead again after revive in veteran Banished Cells I

A few days ago I encountered a weird bug after reviving at the final boss -- I dropped dead again before even restarting the fight. Death recap showed exactly the same stuff that killed me previously, further revive didn't work, had to logout and back in, and then I was not dead (looks like purely client-side bug). I'm curious whether anyone has ever experienced this. I'm not quite sure what triggered it, may have been gear repairs, but I wasn't able to reproduce reliably. Hasn't happened to me in other dungeons so far.

The road to Rilis wasn't flawless as I died a couple times to adds, but also not hard enough to dissuade me from reading the Scroll of Glorious Battle. So Rilis wiped the floor with me a few times, sometimes I revived on spot, sometimes at entrance and ported back -- there I noticed something was off -- the door from the room with Skeletal Destroyer was closed, although I could walk through. After a few failed attempts my gear started falling apart, so I repaired a few pieces, interacted with the Scroll, dropped dead instantly (didn't even get to reading it). Revive not working, quit the game, logged in again, and alive. By that time I think I stopped reading the scroll, still died so again had to repair some armor, swapped weapon and dropped dead. Logout, login, alive. On one occasion (not sure it was after another repair or recharge) I dropped dead right after post-revive ghost form ended and I made a step. Logout, login, alive.

Eventually I did defat him and completed quest, so it was not the whole dungeon bugged. Maybe my soul gems are faulty?
