Vault of Madness conqueror achievement blocked (again)

This has been on and off issue / bug for years - google and will see come up. Seems gets fixed, then broken again.
Tried today with pledge, and non-pledge runs (4x) to try get achievement. It blocks at 2nd boss (sp? Ulguna Soul Reaver). Did runs where I was not lifted up at all, or was lifted - thought maybe being lifted prevented getting kill credit but nope, not the issue.
Normal achievement which requires same boss unlocks just fine - but vet conqueror achieve does not due to this no-credit for killing 2nd boss.
I know there is some odd combo of circumstances that DOES allow completion of this achievement because couple weeks ago when ran this pledge on another char, recall getting same no credit issue, but ran it again and on 2nd attempt, got credit.
This time though, I've given up after 4 repeated attempts.

✭One of the folks in my vVOM pledge yesterday failed to get the Survivor achievement, even though there were no deaths. (I already had it, so, don't know where the 'issue' arose).1
