On "in combat".
We can have "in combat" status randomly, standing on an empty field.
We cannot ride "in combat", we cannot manage skills "in combat", we cannot check mail "in combat", but...

Got to be able to get your crown deadly poison and tri restoration potions.

I know a better option. "Reduce bugs" token! 1 day - 700 cr, 4 days - 2500 cr. You pay and receive a special exclusive message blinking in the right top corner of the screen "All bugs are reduced on your account!"1 -

What were you doing when it happened? Using in add ons that could be an issue? How does this harm your personal gameplay on a daily base? It happens anywhere and this is not about having to get out of combat to switch skills.
I have rightfully been in combat at a keep. Walked away to swap skills, Walked to the next keep, all the way inside the keep and was still in combat yet I had not seen any combat since I left the first keep. That takes minutes of time.
I have been in the middle of nowhere, in sneak with nothing around to initiate combat and was mysteriously placed in combat.
I have died and rezed at a keep that was no being attacked and still in combat. I have done the same and rezed at one of the alliance bases that cannot be attacked and was still in combat.
It was not this bad before Murkmire. Not even close. The sad part is Zos has not commented on this so we have no reason to think Zos is interested in fixing it. It is just like the GF issue, many threads but if this ever comes to a head they will claim they did not know.
