Getting problems with accepting dungeon quests.

So you know when you port into a dungeon and another player has already spoken to the quest giving NPC you would also be given the option to accept the quest? Well I've been having a problem where after I accept the quest that way the quest is not added to my log nor do I have the option to pick it up from the NPC anymore. It also does not progress when we move through the dungeon. I tried asking my groupmates to share the quest but I still cannot add it to my log after accepting it. The only remedies for this seems to be to pray I'm the first one in there to speak the NPC or to leave the dungeon either by being kicked or manually leaving which results in the 15 minute wait. Has anyone else had this problem? For what it's worth trials quests do not have this problem.

You can enter the dungeon without group and pick up the quest, then queue for it.
Has not had this issue myself but did that for the last two dlc just to get an preview of dungeons.
