PS4 draw distant visuals are so pathetic and bad, why?

I have died twice now to guards trying to run away because while I am running not even 2 feet in front of me is loaded up so I run into an invisible guard or get stuck in a group of invisible people and die. It's like this everywhere, citizens and NPC's take a good 5 seconds to appear, why?
