This is happening more and more in pvp. Is it the use of poisons and something else? It happens frequently enough, I wish I could isolate how it happens.
Permafrost it happens with at times, also were wolf howl......may be others I cant remember.

is not new and you are not alone.
im not a dev, but, this 3x break free happening (in my belief) is from lagg.
